About

The Front Porch is a multi-agency, community-based resource center in Chatham County that works with youth under the age of 18 and their families who may be at risk of entering the juvenile court system due to behavioral, dependency, or service-related needs.

The program serves as a voluntary early intervention and diversion initiative designed to connect families with appropriate community resources before involvement in the justice system becomes more serious. After a referral is received, each youth and family participates in an assessment with a case manager to identify needs and strengths.

A coordinated, multi-agency team then develops an individualized support plan, which may include counseling services, behavioral health referrals, mentoring, educational support, and access to basic needs such as food, clothing, and housing assistance. Families are supported through follow-up and case management for approximately 90 days to help ensure successful connection to services and long-term stability.

The Front Porch is a collaborative effort involving multiple community partners, including local government, schools, courts, and social service agencies, all working together to strengthen families and improve outcomes for youth in Chatham County.
